Parent or Guardian of Student Athletes,

1. Log onto Rank One Sport and create an account. The link is: Online Physical Application
2. Complete Health History Form.
3. Complete Athlete Information Form.
4. Complete Athletic Participant Form.
5. Complete Insurance Waiver.
6. You will need to print a blank physical form and take it and your child to the doctor. Return completed form to Nurse Lynn at BMS. Your doctor may also upload the physical. (Click Here for Form)
7. Steps 1 through 6 must be completed BEFORE your student may participate in tryouts, games, or practices. Your student must also be academically eligible.

*In order to successfully link to your student, you must use the student lunch number without zeros in the student id sections.  Your child uses this number daily.
